of Lifestyle Support Workers

Lifestyle support workers must ensure:

Consultation has occurred with the Program Management, Team Leaders, and/or House Liaison, validating that all risk management assessments have been undertaken in relation to any new support delivery, of a service user as identified in their Individual Service Plan


Copies of all documentation of Risk Assessments undertaken, for the support delivery of a service user as identified in their Individual Service Plan, is received by them from the Program Management, and or Team Leader/s and House Liaison Officer/s e.g. (supplied a copy of the Risk Management Plan) for implementation and control

LIFESTYLE SUPPORT WORKERS MUST ENSURE:

}  They supply feedback requirements to management to enable ongoing monitoring, and reviewing by Program Management of any Risk Management Plan.


}  Any new risk / hazard identified either for the service user, or any staff member, or in relation to an activity that the service user, or staff member is associated with, that risk management policy and procedures are followed, and brought to the notice of management. (5 step process) CLS approved Risk Management forms to be used.

LIFESTYLE SUPPORT WORKERS MUST ENSURE:


}  They undertake annual “in-house” CLS training of the service risk management policy and procedures.


}  Consultation with the service Workplace Health and Safety Officer/s occurs as required.
