Some Findings

• The Issue stopped happening since September 2nd
• The Issue started around May 5th
• Looked at the code merged and removed around the ends of August and beginnings of September and nothing easily apparent suggested to be the root cause

Some more findings

• Around 2300 Patient Results were affected
• 70% of the results affected were imported
• the other 30% affected were created manually
• Around 97% of affected results were on Patients of doctors that were still not using the system

More findings to make this even weirder

• 70% of the ocurrences had the same timestamp of sign_off as its created_at

The Good news

• The results that were actually being actively monitored and signed by the TFC team actually did have a signer ID
• Of the patients that were actively tracked only 70 patient results were affected
