- The format of a death certificate is Universal
- Death certificate has 2 parts
1.Medical part: it is for the cause of death and manner of death. It also includes time intervals and ICD coding.
2.Non-medical part : it is for the identification and demographic particulars about the deceased at the top and the identification details of the certifying physician at the bottom

- There are 2 main types of death certificate
a. Institutional deaths to be filled in Form No. 4
b. Non-Institutional deaths to be filled in Form No. 4(A)
- Filling a death certificate includes filling Part 1 and Part 2
Filling a Death Certificate
Each event in the sequence should be recorded on a separate line
- the immediate cause of death is entered on the first line, written in l(a)
- any intervening causes on the lines between the first and the lowest used line, written in l(b)
- the underlying cause of death is entered on the lowest used line, written in l(c)
